“Physical Day for ChMS Athletes- If your child is currently in 6th, 7th, & 8th grade at Chelsea Middle School and interested in athletics, then they must have a current physical on file at all times. Lemak medical group offers an in-school physical each January for our athletic program. Athletes must pick up a blank physical form from Coach Butler for parents to complete the front page. That form must be turned back into Coach Butler along with $20. Your child will then complete their pre-screening during school hours and also attend the doctor portion of the examination on January 19th during school hours. Students will be taken by bus on the 19th to the Lemak Alabaster location to complete that portion. The best thing about this program is that it is cost effective and time efficient for families as everything takes place during school hours. All students planning to participate in this program need to return the physical form with the front page completed by a parent and $20 to Coach Butler by January 11th.”
