Forms for 2013 Season!
Here is a copy of the parent packet that was passed out at 12/12/12′s information meeting/evaluation, as well as the entire print out of the Substance Abuse policy.
Remember, all completed forms are due by Friday, January 11th. PLEASE contact Mr. Wood via email at cwood@shelbyed.k12.al.us with any questions regarding this process.
Fees for the season will be due by the end of January and include:
$165 school participation fee (made payable to OMMS and turned into Mr. Wood)
$130 spirit pack/uniform (made payable to OMMS and turned into Mr. Wood)
$140 clinic fee (made payable to Inverness Racket Club, but turned into Mr. Wood)
