Dear Parent or Guardian, As the end of the school year approaches, I’d like to take this time to remind you to pick up your child’s medication the last week of school. All medications must be picked up and signed out by end of day May 23. According to the Shelby County school policy, all remaining medications not picked up will be destroyed per school policy. The school cannot store any medications for the following school year. As a reminder for the next school year: Alabama State Law requires that all students entering school in the Alabama school systems are immunized against all vaccine-preventable diseases and present an Alabama Certificate of Immunization (IMM-50 form /“Blue Form”) before attending school at any grade level. These forms do not have to “blue” any more. All over the counter medication (OTC) must be in the brand new original bottle/package, with the child’s name clearly marked on the container, with the correct information on the label, and it must be age appropriate. Don’t forget to check the expiration date. OTC medications must be accompanied by an “OVER THE COUNTER” medication authorization and requires a parent’s signature. All prescription medications must be accompanied by a “Prescriber Authorization” and requires a physician’s and parent’s signature. All medications must be signed in at the health room and during the hours of (7:30-2:30). No medications will be allowed to “carry on person” without the Physician’s Medication Authorization form specifying these instructions. You may obtain a School Medication Prescriber/Parent Authorization form on the Shelby County Schools website under the Student Services information and then click the Nursing/Health services tab. There are now two different forms: Prescriber Authorization (requires a physician’s signature, used for prescription meds) and Over The Counter Prescriber Authorization (used for over the counter meds, may require a physician signature if dose on box is NOT age appropriate). Choose the sufficient one that meets your needs. All Health and Medication administration forms must be renewed each new school year.
