SPORTS PHYSICALS

Dear Parents,

This year Lemak Sports Medicine will be conducting physicals for high school students and current 8th grade students. Physicals will be completed in three different stages, online registration/pre participation, pre-screening, and orthopedic evaluation.

• All participants must use the AIRR Parent Portal link and register for physical by filling out the physical form online. COMPLETED BY DECEMBER 20TH
• The athletic trainer will collect the following data on-site at the school (Height/Weight, Vision, Blood Pressure) AT CHHS, FIRST PART OF JAN 2014. DATE TBA
• After the above is completed, the student athlete will need to attend physical day for their orthopedic evaluation to complete the Sports Pre-participation Physical. MARCH 14

The Pre-participation physical registration can be done online by parents and/or student athletes and is due December 20h. Once the registration and physical pre-participation online is complete, pre-screening will be scheduled at the Chelsea HS athletic training room the first week of January, after school. Lemak Sports Medicine has made a mandatory deadline to have all online pre-participation and pre-screening completed by January 15th. Physical day is Friday, March 14th at the Jeff State (Hoover campus off Valleydale road) and will only consist of an orthopedic evaluation.

Instructions for parents/student athletes:

1. Log on to www.Airrsports.com, click red link for parents on main screen.
2. Sign up for Circlebox, enter name, email, and create a password.
3. Click “member’s area” on left side, and scroll down toward end of page. You should see the start of the Medical History of the Physical form. Complete the medical history section and click Save, then Log off.

Other information:
1. Physicals are provided by Lemak Sports Medicine physicians and athletic trainers and are free of charge. Chelsea HS Sports Medicine Program does ask that each parent/athlete pay $5.00 on pre-screening day in order to help with medical supplies, equipment, and upgrading of the athletic training room, Cash or Check payable to Chelsea HS will be accepted.

2. Any questions regarding the disclosure or terms of use during registration need to be directed toward Britney Bates, Director for Sports Medicine Outreach.

Note: Any student who does not comply with this process will not be allowed to participate on physical day. Additionally, anyone who is unable to attend either physical date must obtain their own physical or coordinate an appointment through the athletic trainer and appropriate clinic physician. All physicals performed at LSM clinics will be subject to a $25.00 fee.
