All student-athletes must submit the following paperwork per the Shelby County Board of Education and Oak Mountain Middle School.
Print page 9 of the Shelby County Drug Policy included below, sign, and submit to your head coach.
Print the ‘volunteer driver form’ and ‘permission form car’, fill out, sign, and submit to your head coach. These forms do not require you to transport other students, but we must have these forms on file as parents will transport students to and from matches and tournaments.
Print the OMMS Consent Form, fill out, sign, and submit to your head coach.
