Dear Parents:

Our primary goal at Oak Mountain Middle School is to consistently examine ways to provide the best opportunities for our students. In order to better meet these needs, please take a moment to complete the following brief survey.  The survey results will be used as part of our Continuous Improvement Plan to help us in meeting the needs of our students to the best of our ability.  Once you click the following link, choose Shelby County Schools and Oak Mountain Middle School.
