Wilsonville Civitan Club’s Revis Memorial Scholarship
The application is a pdf file which can be filled out on the computer (preferably) or printed and filled out. The 2-page application and any other documents can be emailed to aj15phelps@gmail.com or mailed to Anne Phelps at 47 Hill St, Wilsonville, AL 35186.
